| When sending my specimen back, is there any special handling required? |

There is no special handling required. With approximately 1 ml of a random spot urine specimen, use your vial and send by overnight mail to:

| I don't have a vial, what should I do? |
A
| You can transfer the specimen to an NCL mailing kit vial. Be sure to send by first class mail ($1.13 postage required). |

| Will I need to refrigerate the vial to make sure it isn't ruined? |
A
| Refrigeration isn't necessary because the NCL vial contains a preservative |

| How long will I have to wait before I know my results? |
A
| The turn-around time is typically 1-3 days. If you want faster service, including a FAX number will help us contact you. |

| What are normal MMA levels? |
A
| Normal MMA is < 3.8 micrograms of MMA per milligram of creatinine (< 3.6 micromole MMA per millimole of creatinine). |

| How much does this test cost? |
A
| The cost is $150.00, net 30 days. For screening or research studies, discounts are available (if specimens are submitted in groups of 4 the cost is $125 each, groups of 20 are $100 each). For more details regarding discounts, please call us at 1-800-397-7408. |

We can bill Hospitals, clinics, and physician offices if prior arrangements are made. Doctors requesting Medicare and/or insurance billing must include a copy of the Medicare insurance card. NCL accepts payments for patients by check, money order, or visa/master card.
